A page for someone you love, at no cost, in a few days.
A Warrior Story honors an OM warrior's journey and gives friends and family a place to remember them, share their story, and give in their name. The Foundation builds the page with you. There is no charge, now or later.

Tell us about them
Use the form below: who they were, their story, photographs, and any links. Fifteen minutes; you can save and come back.
We build the page
Typically one to three business days. If a service is imminent, say so and we move faster. You review a draft before it goes live.
Share the link
A permanent address, omlegacy.org/stories/first-last, for the obituary, the program, or a note to friends.
Gifts in their name
If you choose, the page carries a memorial fund. Every gift is receipted and you receive a donor list to thank people.
Create a Warrior Story
The form asks for your details, theirs, the story in your words (an obituary is fine), photos, and links. Nothing is published until you have approved a draft.

This rare disease does not define an individual. A page can carry any of these sections, in any order, and families can add to them over time:
- Their story: who they were, in the words of the people who knew them
- What they loved: the places, people, work, and small pleasures
- Their OM journey: diagnosis, treatment, the doctors and hospitals that mattered
- In their words: sayings, letters, voice recordings, video
- Family memories: contributions from children, friends, colleagues
- Their legacy: the fund in their name and what it supports
- Full name, the name they went by, and a line of who they were (Wife. Mother. Teacher. Traveler.)
- Dates of birth and passing, if you would like them shown
- A remembrance of any length, or an obituary
- Photographs, the originals if you have them (phone photos are fine)
- Links to videos or articles; voice memos and audio are welcome
Every tribute page from the Foundation's original site has moved here as a Warrior Story, and the old links forward to the new address. To update photographs, add to the text, or add a memorial fund, use the form and say which page it is for, or write to us.
